Outage Support Requests

Report or request updates regarding system or service outages, including network, phone, cloud, or application disruptions.

Before you begin

Use this form to report any service interruption or performance issue affecting systems, applications, or technology services.
Please include as much information as possible, such as the system name, location, time the issue began, and a brief description of the impact (e.g., users unable to log in, slow performance, etc.).

Available Request Types

  • Network – Report issues affecting wired or wireless connectivity.
  • System – Report outages or degraded performance in servers, applications, or internal systems.
  • Cloud – Report access or performance issues related to cloud-based services.
  • Phone – Report disruptions with telephone lines, voicemail, or call quality.
  • Colleague – Report problems accessing or using the Colleague system.
  • AV – Report issues with audiovisual equipment, classroom displays, or event technology.
