What is it?
Requests entered for continuing ed students seeking to transfer to another course, get a refund for a course or cancel their registration in a course.
Before submitting, please note:
- Be sure to complete the Course Cancellation or Addition request form before submitting this student change form.
- Late Withdrawal requests are subject to review by Student Records and a separate review committee. Submission of this request will direct the student to complete the late withdrawal supporting documents and does not start the review process until after it has been received by the committee.
- Be sure to follow the CEED Withdrawal/Refund/Transfer policy and only submit those requests that follow the policy as written.
